How can I reduce this stress in my life?

It’s just been made very clear to me that I need to reduce the stresses in my life soon or my health will suffer. After nearly passing out a little while ago after waking up in the middle of the night, I had to look up the reason for it. I was seriously worried about the possibility of that being a heart attack. And now that I have looked it up, signs seem to point to anxiety. I’ve experienced a lot of the symptoms even before now. I’m very much convinced that it’s because I have so much stress in my life, and while before I would suck it up and shrug it off, I know now that it should not be taken so lightly! I’m a teenager, 16 and a junior in high school. I have worries about my future. I don’t feel comfortable in my own skin. My home life’s a little twisted and has been for most of my life. I just broke up with my boyfriend who I was very close to. I have almost no friends that I can relate to or rely on. And although I’m fairly intelligent, I still have problems with school/grades. With my whole life in general, I feel inadequate, and asking for people to cut me some slack has never really seemed like an option. I always have to be strong with things, and when I have had to ask for breaks from things (like days off school from my mother), I feel like I’m being seen as weak or lazy. But now I know that I need to find out some way to ask for help. I just don’t know what to say or do. My family can’t exactly afford therapy, doctors, or drugs right now, but how exactly do I at least go about telling people about my need for relief and asking for help with it? Is it even okay for me to ask people to cut me some slack? Do I deserve it in these times when everyone else seems to have their own problems to worry about?
